Personal Information Form - By providing the information requested in this form, you will make your membership at Trinity Church more personal and will enable the church to serve you better.

Personal Information Update Forms - Use this series of forms to update personal information that may have changed, such as phone number, email address, street address, etc.

Prayer Request Form for Sunday Bulletin  - Use this form to submit a name to be included on the Sunday bulletin prayer list.

Prayer Request Form for Daughters of the King - The Daughters of the King also maintain a prayer list. Use this form to be included in their daily intercessory prayers.

Sexton Services Request Form - This form can be used to make a request of our sexton, report a problem, ask a question or offer a suggestion.

Sunday Server Form - Use this form to place your name on one of the Sunday Server lists (bakers, greeters, oblationers, etc.) or to remove your name from one of the lists.

Transfer Request Form - Use this form to initiate a transfer of church membership from your current church to Trinity Episcopal Church.
